CHAMP CAMP ENROLLMENT CHECKLIST
Submit ALL required registration documentation:- Complete, sign and submit 2018 CHAMP CAMP Registration Packet
- Complete, sign and submit an updated Camper Health History Form
- Complete, sign and submit Medication Administration Authorization Form
- Early registration payments can be made online via PAYPAL
- Weekly fee payments and before care and after care can be made via PAYPAL
